Please ensure that the form is completed correctly, incomplete applications cannot be processed and will be returned to sender. Post or fax your completed, signed application form together with copy of your deposit slip. (Proof of payment is required WITH application)
Please DO NOT send copies of Certificates. The Application Form is a legal document so your main qualification should be stated on this document. Should there ever be a claim against you, you would be required to produce your qualification certificate.
We will organise immediate insurance cover for you and you will receive confirmation through the post. (If you would prefer us to fax confirmation to you please request this with your application, giving clear details and fax number).
We will register your application with our broker based on the information provided in your Proposal Form. Please note that should your CPR or qualification certificate expire during the term of this insurance the onus is on you to renew / validate your certificate with the relevant institution.
